The Master's degree Program Proposal form is a university and department milestone requirement. This form must be signed and approved by the department's student services administrator before submission to the student's program adviser. This form is due no later than the end of the first quarter of enrollment in the program.

Request for an extension of Masters program
This form is required of students who need to petition to extend their Masters program beyond the expected two years to completion.
Explain why degree program was not completed within the original candidacy period (5 - 6 quarters typically). Departments are not obliged to grant an extension.
Extensions require review of academic progress and any other factors regarded as relevant by the department, and approval by the department; such approval is at the department’s discretion.
