Choose one of these two forms to request a Sequoia Hall, CoDa, or Registrar-owned room for your event.

Use of these forms is restricted to Statistics and Data Science affiliates only. Requests submitted by those outside the department will be denied. (Other users can contact administrators in their specific department or program for help with room reservations!)
